PRACTICAL MANUAL OF HIGH-QUALITY GUIDANCE WITH NUTRITIONAL AND DIETOTHERAPEUTIC FOCUS

Abstract

Numerous patients leave the hospital in precarious nutritional status and low post-discharge nutritional care is observed. With this, the aim of the study was to develop guidelines for caregivers and patients at hospital discharge. The work was carried out based on literature reviews in the databases and the language for the description of the content was adapted, without technical terms and easy to understand for the lay population. The pathologies chosen were stroke, hypertension, cancer, diverticulitis, anemia, vitamin K deficiency, gout, renal failure, obesity, postoperative gastrectomy, burns of 1, 2 and 3rd degree, gastroesophageal reflux and peptic ulcer. Practical interventions are necessary to improve the nutritional status of the patient before, during and especially after hospital discharge. Post-discharge treatment offers a huge opportunity to optimize the quality of life of individuals, improving their nutritional clinical outcome. This manual provides new insights into post-high nutritional and dietotherapeutic care.
